Fear of falling in love, I think this shot was nicely put together. Based on the tears you recreated it conveys that youre afraid to show feelings and just on that alone its safe to say you did a great job expressing what your phobia is. Very well done!
To me this is the fear of peer pressure, pressure to be cool and do what others want, or feeling different and needing to validate yourself. This is a pretty great photo, i do think it would have been interesting if you had put the name of the fear behind you, maybe selecting yourself and putting the text in the layer behind you or putting it on the sheet itself. This really is a great photo.
The fear of toxic situations/people. You did a really good job representing this with the hands on the back side of the sheet and with the lighting one the back ground. One thing I would change is the lighting on your subject (brighter)

I think it's the fear of peer pressure as well, a lot of good details in this photo the darkness, the emotion you feel from the piece and the T-shirt was a even greater touch to symbolize wanting to be accepted. I think one thing you could've is blend your phobia in the photo but it doesn't really matter overall it looks good.
